Resolved Alert:

As of 10:30am this morning Dynamic Forms platform is functioning normally. No new tickets have come in and other institutions are reporting normal operation. We have confirmed that the platform is fully functioning and back to normal.

The issue was first reported around 6:30am this morning but was intermittent and could not be verified. Around 8 am we received multiple tickets. Contacted Dynamic Forms support and they responded that they are aware of the issue.

We are still waiting to hear back from Dynamic Forms support as to what caused today’s slowness.

Dynamic Forms platform has been having intermittent login / submission issues since early this morning. We have opened a ticket with their support team. We have confirmed that other institutions have similar issues. Users may need to try multiple times to login right now.
