Dynamic Forms Login Issues
April 10, 2024 1:30 p.m.The issue was first reported around 6:30am this morning but was intermittent and could not be verified. Around 8 am we received multiple tickets. We contacted Vendor support and they responded that they are aware of the issue.
Login issues and slow response when trying to reach Dynamic Forms.
Any student, faculty, staff who submits a request through Dynamic forms platform could have been affected.
From the vendor: This morning unexpectedly Azure created several new keys for each service which meant as users were logging in passing from one Dynamic Forms Azure property to the next, the key was different, not recognized and the session was dropped.
To resolve in the short run, the vendor removed the extra keys so we were back to a single key and users where recognized as they went through their session.
Vendor is working with Microsoft on how to prevent future issues.
