ROOT CAUSE ANALYSIS
Cause:
The power outage prevented the switch from coming back.
Resolution:
Reset the supervisor blade.
Prevention:
The power outage was due to defective power outlets, which are outside the purview of ITS; however, we plan to have enough spare parts on hand and personnel on-site to respond quickly to outages.
