ROOT CAUSE ANALYSIS
Cause: 
  • Bad configuration for website within hr.conf
  • Configuration file was point to …/hr, which doesn’t exist. Configuration was updated to …/HR/web and Apache was restarted to resolve the issue.
  • Restore file from March 1st and file was the same as earlier today. File was dated Feb 7th, which would have been the last configuration change.
  • Issue didn’t show up sooner as apparently Apache was not restarted and therefore didn’t pick-up the changes. The Linux patches applied yesterday, March 31st, forced a restart of Apache.
Resolution: 

Configuration was updated to …/HR/web and Apache was restarted to resolve the issue.
