Initial Symptoms
University VPN using DUO was not working.
Root Cause Analysis
Cause
The original configuration of DUO proxy servers talking to the domain controllers via the F5 load balancer stopped working for some as of yet unknown reason.
Resolution
The DUO proxy servers were reconfigured to bypass the F5 load balancer and instead directly talk to the domain controllers
Prevention
Have multiple proxy servers configured as fail-over in case one fails.
