Root Cause Analysis
Cause
A power supply failed, as a result a new power supply had to be installed.
Resolution
A replacement power supply was installed, but had a different plug style, so Facilities had to install a new outlet, thus adding to the outage duration.
Prevention
Equipment failure, nothing could have been done.
