Service Summary

Dynamic Forms (DF) provides a simple way for non-technical users to take any paper-based form and quickly build an electronic form that can be shared with a link. This secure e-sign/workflow solution allows users to focus their time and energy on helping students, and not on manually processing documents.

Getting This Service

Select Request Dynamic Forms Account to:

  • Open or close a Dynamic Forms account
  • Add a new org
  • Add or remove users from an org
  • Technical Support
    • Ask a technical question
    • Help setting up a new Dynamic Form
    • Log in issue
  • Other question/issue

Availability

This service is available 24/7, excluding planned outages, normal maintenance windows, and unavoidable events. Maintenance windows are Sundays from 7 to 11 a.m. but may be extended to 2 p.m. if needed. If maintenance is required outside of these hours, it will be announced on the Alerts & Bulletins page.

Training

Training is available upon request. Select the Request Dynamic Forms Training then select Other. In the Request Description box, specify "training" and for how many people.
