Any Mason user dialing an off-campus line or 10 digits on Mason heard the following message:  Your call cannot be completed as dialed. Connections were never made.

Initial symptoms: 

Switchboard notified teladmin of issues they were having. Teladmin tested and confirmed.

Impact to Mason: 

All Mason Vonage phone users were affected trying to make or receive off campus calling. Users were able to dial internally with 5-diget dialing from the Vonage app, phone and teams integration.

ROOT CAUSE ANALYSIS
Cause: 

At approximately 12:22 pm ET on Wednesday, October 15, 2025, our internal monitoring systems alerted us that some customers were experiencing an intermittent inability to make or receive calls. Although we operate a redundant environment across two geographically diverse regions, with active failover capabilities in place, during this incident, call failures and service degradation were observed across both sites.

Resolution: 

Mitigation actions, including an increase in call capacity and a temporary change to limit repeated call attempts, were implemented. This alleviated network traffic, and service was restored by 5:23 pm ET that same day.

Our engineers are actively working on a comprehensive root cause analysis of this incident.
